New Patients
Your First Visit
Your first visit with us typically takes about 45 minutes. During this time, we’ll review your dental and health history, talk through any concerns or goals—such as cosmetic dentistry, jaw pain, teeth grinding, or snoring—and complete a thorough exam of your teeth, bite, jaw, and soft tissues. Digital X-rays will be taken, and if needed, we may also capture photos or models of your mouth to ensure a full understanding of your oral health. After your exam, we’ll schedule a personalized hygiene visit and a consultation with Dr. Kellee to discuss treatment options tailored to your unique needs and preferences.
Registration Forms
Your time is valuable, so you can download new patient forms here. Simply complete the forms at your leisure, then bring them to your initial visit. If you prefer, we can either fax or send them to you so they can be conveniently done prior to your appointment.
Dental Insurance, Payment, & Financing Options
Our team is happy to work with you to maximize your insurance coverage. We process and file claims for most PPO insurance plans, and we’ll review policy information to help you complete claims for other providers. Don’t hesitate to ask any questions you may have about your benefit plan. Our knowledgeable team will do their utmost to answer your questions.
We accept cash, checks, and credit cards, including American Express, Discover, MasterCard, and Visa. We accept most major insurance plans, and as a courtesy, we will electronically file your claim for you. Please be prepared to pay your copayment, deductible, or full visit charge at the completion of your visit. We offer a 5% cash savings with payment in full via cash or check on the day of treatment.
